HAVE THE FOLLOWING DOCUMENTS READY

Residence History: Your complete residence history for the last 2 years. Make sure that you have the name and phone number for your landlord and your mortgage company.

Employment History: The name and addresses of the companies that you have worked for in the past 2 years, and the dates that you worked for them. Be prepared to document the reason for any job gap longer than 1 month. *You will need your 2 most recent pay stubs and your last 2 years w2 forms

Self Employed?: You will need your last 2 years individual tax returns and a current YTD profit and loss statement if you are a sole proprietor. If you are incorporated you will need your individual returns, any applicable W2 forms, and your corporate returns for the last 2 years, as well as a YTD profit and loss statement. Please include all schedules

Bank Accounts: The names and addresses of the financial institutions where you maintain your checking/savings or investment accounts such as mutual funds or money markets. Please provide the 2 most recent statements for each account.

Retirement Accounts (401k): Please provide your 2 most recent statements for any retirement accounts such as 401k accounts, traditional IRA, Roth IRA, college funds etc.

Life Insurance: The amount of life insurance that you have and the cash value of all policies that you are holding.

Other Assets: List the other assets such as the year, make and model of your automobile(s)

Liabilities: List all and include: creditor names, addresses, account numbers, monthly payments and balances. If you have recently opened an account what did you purchase? If you pay alimony or child support, please include it in your liabilities.

Misc: Your homeowners insurance agent’s name, address and phone number. Provide your most recent declaration page from insurance company. If you are divorced please provide your divorce decree from the courts. If you pay child support please provide the child support papers from the court.
